If you are a new patient, we request that the New Patient Registration forms be completed prior to your visit with Dr. Allen or Dr. Lin. These forms can be found (in pdf format) by visiting the following link: Allen&Lin/NewPatientInformation.pdf or at our office prior to your first visit.
Along with the New Patient Registration forms, please bring your insurance card.

Dr. Allen and Dr. Lin are on call 24 hours a day. They may be reached at (760) 564-3533 during and after office hours. If you have a medical emergency, please dial 911 immediately or go to the nearest emergency room.
Our office sees patients according to appointments. We also understand that there may be a need for urgent visits; we ask that you please call before arriving in order to accommodate you with the utmost efficiency. Please understand that if you call prior to your arrival you may still be scheduled between other patients.
We ask that prescription refill be requested 24 to 48 hours in advance. Please phone your pharmacy with your request, and they will contact our office.
We request that co-payments and office visit payments are expected at the time of service.
