The Health Information Portability and Accountability Act of 1996, also known
as HIPPA Notice of Privacy Practice provides the patient with more control over
how his/her information is disclosed and used.  The HIPPA Privacy Rule
became effective as of April 14, 2003 and has created uniform federal privacy

Scheduling
We are accepting new patients that are related to currently established
patients at this time. Please call our office to set up an initial appointment.  

We offer same day appointments for patients who have an acute illness.  If
you need to make a same day appointment please cal our office in the
morning.  For less urgent needs, please schedule your visit ahead of time.

In preparation for your first appointment, please bring a list of your present
medications, your medical history, immunization records, and your insurance
card. You will be asked to fill out a patient history form upon your arrival.  
Please arrive 10-20 minutes prior to your first visit to complete the paperwork.

Billing
We are the preferred providers of most PPO’s, and we do accept Medicare.  
In addition, we are also the preferred provider of OASIS IPA which serves the
following HMO plans: Aetna, Blue Cross California, Blue Shield, Health Net,
Kaiser, Pacificare/Secure Horizons, Scan, Universal Care.

We are more than happy to bill your third party insurance.  If you have any
questions about your insurance, please feel free to contact our office.  Our
office uses an outside billing service to process insurance billing.  If you have
any questions regarding how your insurance is billed please contact
Professional Health Care Billing Services at (760) 778-1992.

Co-Payment is expected at the time of service. We accept cash,
checks and major credit cards.
Prescriptions
A prescription refill is a prescription on file with your pharmacy that has
remaining refills. This prescription can be refilled without contacting your
provider.

A prescription
renewal is a prescription on file with your pharmacy that does
not have any remaining refills. The provider must be contacted for approval
before the prescription can be refilled. The renewal will usually allow for
several refills. In most cases, we can renew medications without a new visit if
you have been seen for this problem within the past year. The provider may,
however, request that you be seen more frequently to allow for proper
evaluation and documentation of your condition and your response to the
medication.

If you need a prescription renewal and have seen your doctor within the last 6
months, contact your pharmacy to fax the prescription to our office
(760-564-3360).  We will renew most of these prescriptions within 24 to 48
hours.
Labs
Lab work can be done at either LabCorp or Unilab.  From previous
experience, we prefer to use these sites as they offer the most efficiency in
returning your blood tests in a timely fashion.

Release of Records
If you wish to have your records transferred to our practice, contact our office
and request and sign a release authorization to be sent to your present
provider. To save time, this form can also be downloaded, printed, filled out
and sent to your present provider. If you wish to transfer your records from our
practice, please contact your new provider and they will request the records
be transferred. To save time, this form can also be downloaded, printed, filled
out and sent to us.
